Transition Planning   

Preparing Students for Life After High School

Transition planning is an important part of the Individualized Education Program (IEP) for students with disabilities. Under the Individuals with Disabilities Education Act (IDEA 2004), transition services are designed to help students prepare for life after high school.

The transition process focuses on each student's individual strengths, interests, preferences, and goals. Through thoughtful planning and collaboration, students, families, educators, and community partners work together to identify the supports and resources needed for a successful transition to adulthood.

Our Goal   

Livingston Parish Public Schools is committed to helping students with disabilities prepare for meaningful and successful lives after high school. We strive to connect students and families with resources, programs, services, and educational opportunities that support each student's transition from school to post-school activities.

Transition planning may include preparation for:

When Does Transition Planning Begin?  

Transition planning begins during the student's IEP process and is based on the student's individual needs, strengths, interests, and goals. The IEP team works with the student and family to develop measurable postsecondary goals and identify appropriate transition services and activities.

Students and families are encouraged to actively participate in the transition planning process and begin exploring future goals early.

Families and Students: Your Voice Matters  

Students and families are important members of the transition planning team. Your knowledge of your child's strengths, interests, preferences, and needs helps the IEP team develop meaningful goals and services.

We encourage families to ask questions, explore available resources, and work with their child's school to create a transition plan that supports the student's vision for the future.
